“… i medici sanno parlare però non sanno ascoltare…”, così diceva Nanni Moretti nel film "Caro diario", e questo potrebbe essere il titolo di questa recensione, che non valuta né giudica il Dott. Giuseppe Curinga per le sue competenze nel campo della chirurgia plastica ed estetica, ma valuta la persona in quanto tale ed il suo rapporto con i pazienti, e nel mio caso specifico lo solleva da ogni tipo di responsabilità professionale relativa agli interventi eseguiti in questi anni. Il Dott. Curinga, negli ultimi quattro anni, si è trovato suo malgrado a dover confrontarsi con una rara patologia infiammatoria della pelle, il pioderma gangrenoso che provoca ulcere dolorose e tende a peggiorare anche dopo piccoli traumi; senza test specifici né una cura definitiva. Le terapie più indicate (cortisone, immunosoppressori, terapie biologiche) non sono praticabili senza rischi elevati nel mio caso, a causa di un quadro clinico complesso: glaucoma, trapianto di cornea e tumore al rene destro. L’approccio chirurgico non rappresenta la soluzione per la cura di questa patologia (argomento molto controverso), Infatti ho sempre chiesto io di chiudere le ulcere, ed il Dott. Curinga me lo ha sempre sconsigliato, dimostrando di non capire il momento particolare della mia vita che sto vivendo – come la perdita della vista, o le complicazioni per un tumore maligno. Il Dott. Curinga non ha mai creduto a questa diagnosi, e mi ha sempre trattato con estrema superficialità anche dopo l'esito dell’esame istologico che ha confermato il pioderma gangrenoso. Non si è mai documentato sulla patologia né ha elaborato una strategia. Col pioderma gangrenoso l’esito di un intervento chirurgico è sempre incerto, infatti non ho mai attribuito la colpa al Dott. Curinga quando gli interventi non riuscivano, poiché per ammissione dello stesso durante un intervento è difficile valutare con precisione quanto estesa sia la parte infiammata o necrotica da asportare. La questione ruota attorno a una frase ripetuta costantemente da lui e dalla sua fidata segretaria Irene (una Miss Minci in chiave moderna, per gli amanti delle anime giapponesi): “La chirurgia non è l’approccio corretto per trattare questa patologia.” Concetto chiarissimo, che sono stato io per primo a ribadire: la chirurgia non è risolutiva, ma resta, per me al momento, l’unica via praticabile per poter affrontare viaggi e interventi necessari alla gestione delle mie patologie. In sala operatoria, si sa, non si entra con ferite aperte. Loro la ripetevano solo per sollevarsi da ogni responsabilità cosa che – del resto – ho già chiarito all’inizio di questa recensione. Constatando la sua stanchezza e il disorientamento nella gestione del mio caso, gli ho chiesto di intervenire sulle ultime ulcere per poi congedarlo. Emblematica la sua frase in sala operatoria: “Io non ne capisco nulla di occhi, però se risolvi questi problemi con la pelle, risolvi anche quelli agli occhi.” Parole che, dopo quattro anni, dimostrano quanto non abbia mai davvero compreso il mio quadro clinico. Mentre scrivo questa recensione, ho un buco aperto sulla spalla, esattamente nel punto in cui è stato fatto l’esame istologico. L’ultimo intervento non è andato bene. Quando ho scritto alla sua fidata segretaria Irene, la risposta è stata il solito ritornello: “Mi dispiace molto per la tua situazione, ma la chirurgia, come ti ha spiegato più volte Giuseppe, non è l’approccio corretto per gestire questa patologia.” Il messaggio si è concluso così. Nessun invito a tornare in clinica, nessuna richiesta di vedere la ferita. A chi legge questa recensione, lascio fare le sue conclusioni. Non rinnego assolutamente l'aspetto economico. Tutte le operazioni sono state pagate, anche quelle con esito negativo. Ma “non bisogna fare di tutta l’erba un fascio”: grazie a Simona e Sabrina per la loro professionalità, le cure e le coccole in questi quattro lunghi anni. Aggiornamento: L’esame istologico l’ho fortemente richiesto io, non voi. Irene, sono stato io a dettarti cosa scrivere sul campione di pelle da analizzare, ricordi ?
Translated: “… doctors know how to speak but they don't know how to listen…”, this is what Nanni Moretti said in the film "Dear Diary", and this could be the title of this review, which does not evaluate or judge Dr. Giuseppe Curinga for his skills in the field of plastic and aesthetic surgery, but evaluates the person as such and his relationship with patients, and in my specific case relieves him of any type of professional responsibility relating to the operations performed in recent years. Over the last four years, Dr. Curinga has found himself having to deal with a rare inflammatory skin disease, pyoderma gangrenosum, which causes painful ulcers and tends to worsen even after minor traumas; without specific tests or a definitive cure. The most suitable therapies (cortisone, immunosuppressants, biological therapies) are not practicable without high risks in my case, due to a complex clinical picture: glaucoma, cornea transplant and right kidney tumor. The surgical approach does not represent the solution for the treatment of this pathology (a very controversial topic), In fact, I have always asked to close the ulcers, and Dr. Curinga has always advised me against it, demonstrating that he does not understand the particular moment in my life that I am experiencing - such as loss of sight, or complications from a malignant tumor. Dr. Curinga never believed in this diagnosis, and always treated me with extreme superficiality even after the outcome of the histological examination which confirmed pyoderma gangrenosum. He never read up on the pathology or developed a strategy. With pyoderma gangrenosum the outcome of a surgical operation is always uncertain, in fact I have never attributed the blame to Dr. Curinga when the operations were not successful, since by his own admission during an operation it is difficult to accurately assess how extensive the inflamed or necrotic part to be removed is. The issue revolves around a phrase constantly repeated by him and his trusted secretary Irene (a modern Miss Minci, for lovers of Japanese anime): "Surgery is not the correct approach to treat this pathology." A very clear concept, which I was the first to reiterate: surgery is not conclusive, but remains, for me at the moment, the only practicable way to be able to undertake the travel and interventions necessary for the management of my pathologies. As we know, you don't enter the operating room with open wounds. They repeated it only to relieve themselves of any responsibility, something which - after all - I have already clarified at the beginning of this review. Noting his tiredness and disorientation in managing my case, I asked him to intervene on the last ulcers and then dismissed him. His phrase in the operating room is emblematic: "I don't understand anything about eyes, but if you solve these problems with the skin, you solve the eye problems too." Words which, after four years, demonstrate how much I have never really understood my clinical picture. As I write this review, I have a gaping hole on my shoulder, exactly where the histological examination was done. The last surgery didn't go well. When I wrote to his trusted secretary Irene, the response was the usual refrain: "I'm very sorry for your situation, but surgery, as Giuseppe explained to you several times, is not the correct approach to manage this pathology." The message ended like this. No invitation to return to the clinic, no request to see the wound. I'll let anyone who reads this review make their own conclusions. I absolutely don't deny the economic aspect. All operations were paid for, even those with negative results. But "we shouldn't lump everything together": thanks to Simona and Sabrina for their professionalism, care and cuddles over these four long years. Update: I strongly requested the histological examination, not you. Irene, it was I who dictated to you what to write on the skin sample to be analyzed, remember?
Caro Gianpiero, siamo amareggiati nel leggere queste parole dopo tantissimi anni di cure. Questo è un ambulatorio chirurgico. Hai eseguito numerosi trattamenti. Hai avuto anche la diagnosi istologica corretta. Ed hai avuto pure da anni l'invito a non eseguire più interventi chirurgici in quanto non risolutivi della tua malattia. Pertanto non comprendiamo il tuo sfogo e ti invitiamo come abbiamo già fatto a seguire un centro specialistico per le patolgie infiammatorie, augurandoti di guarire il prima possibile.
